Order an Official Transcript

- Be aware that requests for transcripts to be mailed (not including overnight requests) may take up to 5 business days to process after receipt.

- CAS and AMCAS applicants, please note that CAS and AMCAS will maintain regular operations, but recommend students have their transcript delivered electronically, rather than by mail.
Free Transcript Request
If you qualify for a free transcript and require it electronically, please complete the Free Transcript Request Form and upload it in your order as an attachment. If you need to submit other forms with your transcript, you may upload them as a pdf into your order, as well. If you forget to upload the attachment into your order, contact Registrar@cookbookss.com to cancel your order so that you can resubmit it with the free transcript form attached. We must have the attachment in the order, so that our processors know to reduce your transcript fee of $9.00 before your credit card is charged. You will be charged a $2.50 processing fee.
Fees and processing time:
- Effective August 1, 2022, 当亲自订购时，官方成绩单的价格为12美元, or $9.50 + $2.90 processing fee (total of $12.40) when ordered through the National Student Clearinghouse.

The official transcript prints a student’s Primary Name from the student information system; it does not print the student’s Preferred Name.
If you need to order a transcript to upload to a website, you may order either a paper or electronic transcript, if eligible.
